The one I've seen in restaurants is like a stand/bracket, with a sturdy foot at the bottom and a solid back. From the back, close to the bottom, a sort of spike sticks out toward you; this is where you stick the triangular wedge of cheese (on the upward point of the spike with its widest part down). Then at the top there are two wing-like pieces sticking out to either side, and the heating elements are inside these. When these are lowered, they expose the angled sides of the cheese wedge directly to the heating elements, causing the cheese to melt and bubble. When it's ready you can then raise the wing and scrape off the melted cheese with your scraper onto your plate. Because there are two wings, it's designed to be placed between two diners sitting side by side.
